Every pick is in our $ or $$ band (under $20) except the Fisher pens and the Parker Jotter, which sit in $$$ ($20–75).
Pentel markets EnerGel and Zebra its Sarasa Dry as quick-drying; JetPens timed the Sarasa Dry smear-free in 2 to 3 seconds on Rhodia paper.
What are the best pens for nurses?
On published specs, the best pens for nurses are retractable pens with a metal clip and an ink that holds on labels: the Zebra F-701 and F-301 in steel, the uni-ball Jetstream RT, and a Fisher Space Pen for charts held upright. A nursing pen lives in a pocket, gets clicked hundreds of times a shift and writes on tape, labels and carbonless forms. The F-701 is #1 because it’s steel with a knurled grip and a metal clip in our $$ band, and its F-Refill also fits the F-301.
Best overall:Zebra F-701. Why it’s here: heavier steel than the F-301 (about 21 g against 11 g) with a knurled grip, sharing its 0.7 mm F-Refill.
Best cheap pen to lose:Zebra F-301. Why it’s here: the same steel barrel idea and refill at 11 g in our $ band, so losing one costs little; 0.7, 1.0 and 1.6 mm.
Best for labels and tape:uni-ball Jetstream RT. Why it’s here: low-viscosity oil-based pigment ink that grips glossy stock and dries fast, where water-based gel can skip.
Best for upright charts:Fisher Space Pen Bullet in a pocket, or the retractable Fisher AG7. Why it’s here: their pressurized PR4 refill is the only one here that writes at any angle.
Best for color-coding:uni Jetstream 4&1. Why it’s here: the only multi pen, black, red, blue and green plus a 0.5 mm pencil in one body.
Best quick-dry gel:Pentel EnerGel RTX and Zebra Sarasa Dry. Why it’s here: the gel options, both marketed as smear-resistant, for charting at a desk.
Best for a lab coat:Parker Jotter. Why it’s here: the classic metal pen here, a 128 mm steel click pen with the arrow clip, in production since 1954.
How did we choose these picks?
We compiled these picks from published sources, not hands-on testing: makers’ specs and ink claims, retailer listings, US availability and price band. Every pick had to be refillable, click rather than cap (except the Fisher Bullet), and come in black, blue and red where color matters; we haven’t worn one through a shift.
How do the pens compare?
Four picks sit in our $ band (under $5), two in $$ ($5–20) and three in $$$ ($20–75). Lengths and weights are the makers’ or retailers’ figures from each guide.

Which ink writes on labels and tape?
Oil-based and hybrid ballpoint inks, and the pressurized Fisher refill, hold on glossy labels where water-based gel can skip. Jetstream ink is an oil-based pigment ink with about a fifth of the usual viscosity, per Mitsubishi Pencil, so it writes smoothly but dries like a ballpoint. Fisher’s PR4 cartridge is pressurized with nitrogen and writes upside down and on damp paper, which is why a Space Pen suits charts clipped to a wall or a bed rail.
Gel pens suit paper charts and carbonless forms where you want a dark, even line. The EnerGel and Sarasa Dry are marketed as quick-drying, and the Sarasa Dry has a timed test behind it; our pens that don’t smudge guide goes deeper.
What makes a pen last a shift?
A metal clip, a click mechanism and a refill you can buy anywhere. Caps get lost in pockets, so every pick but the Fisher Bullet retracts. The Zebra F-701 and F-301 share one F-Refill, the Jetstream RT takes SXR refills sold in every office store, and the Parker Jotter has used the same Quinkflow format for decades.
Weight is a trade-off: the 21 g F-701 and 31 g AG7 feel solid but pull a thin scrub pocket; the 11 g F-301 and 9 to 10 g Jetstream disappear in it. For a shared pen, a cheap steel Zebra is easier to replace than a Fisher.
Which pen for which job?
Charting at a desk: any gel or hybrid; labels and tape: Jetstream or Fisher; color codes: the 4&1. A nurse who writes on IV bags and tape gets the Jetstream RT or F-701. One who charts on a clipboard held against a wall gets a Fisher. One who color-codes medication times gets the Jetstream 4&1, whose clip selects the color so nothing else needs to leave the pocket.
Frequently asked questions
What pen do most nurses use?
Steel click pens such as the Zebra F-701 and F-301 and gel or hybrid pens like the Pilot G2, Pentel EnerGel and uni-ball Jetstream come up most in nursing threads we read. We have no survey data; the picks above are chosen on published specs for clip, refill and ink.
What pen writes on medical tape and labels?
A ballpoint or hybrid ink. Oil-based inks like Jetstream and the pressurized Fisher refill grip glossy label stock better than water-based gel, which can skip or smear on coated surfaces. Pigment gels such as the EnerGel and Sarasa Dry are the quick-drying middle ground.
Is the Fisher Space Pen worth it for nurses?
If you write on charts held against a wall or on damp paper, yes: Fisher says the PR4 refill writes upside down, underwater and from −34 °C to 121 °C. For desk charting, a US$2 steel Zebra does the same job for less.
Which pen is best for color-coding charts?
The uni Jetstream 4&1 holds black, red, blue and green ballpoints plus a 0.5 mm pencil in one 148.8 mm body, so one clip carries five tools. The Pilot Hi-Tec-C Coleto and uni Style Fit let you choose colors.
Are brass or steel pens antimicrobial?
Copper alloys such as brass have registered antimicrobial claims in the US, but pen makers rarely test or certify their barrels, and a steel barrel has no such claim. Treat a metal pen as durable, not sterile, and wipe it like any other equipment.
What is the cheapest reliable pen for a hospital?
The Zebra F-301 and uni-ball Jetstream RT both sit in our $ band (under $5), take refills, and have a metal or sturdy clip. Buying a 12-pack keeps losses cheap.
Do gel pens smear on charts?
Ordinary gel ink can smear for several seconds, which matters when you write fast and close the chart. Pentel markets EnerGel and Zebra the Sarasa Dry as quick-drying; JetPens timed the Sarasa Dry smear-free in 2 to 3 seconds on Rhodia paper.
